*MUST HAVE VALID FINGERPRINT CLEARENCE CARD* *SHIFT: Monday:* *6:00am-6:00pm* *Tuesday: 6:00am-6:00pm * *Wednesday 6:00am-6:00pm* *TITLE OF POSITION:* DTA Direct Care Worker/Driver *LINES OF AUTHORITY:* This position reports to the DTA, DTA Assistant Director and or the DTA Director for administrative duties.
*MUST HAVE VALID FINGERPRINT CLEARENCE CARD* *POSITION SUMMARY:* Employee is responsible for the direct care of members including compliance with the DTA rules and schedules, participating in food services, addressing individual needs of members throughout the shift, transportation to and from home/activities, assistance in the self-administration of medication, participating in the admission/discharge of members, conducting community meetings and members in bathroom, daily maintenance including cleaning of the DTA.
Must possess and maintain a current AZ Driver’s license and are responsible to report any changes in their driver status if/when applicable.
There is absolutely *NO CELL PHONES ALLOWED AT WORK, & NO SLEEPING*.
Employees must maintain uniform 100% when on shift each day.
*PRIMARY FUNCTIONS AND RESPONSIBILITIES:* · *Facilitates the admission and discharge of members.
* · *Complies with DTA schedule, rules, policies, and procedures.
* · *Assists in the self-administration of medication.
* · *Responsible for implementing daily activity and DTA schedules.
* · *Transports members to scheduled program activities/events, outings, local agencies as deemed on their treatment plan and when applicable unscheduled activities/events.
* · *Responsible for participating and providing oversight in all transportation venues.
* · *Responsible for the safety of the member in and out of the vehicles* · *Responsible for documenting daily progress notes when applicable.
* · *Responsible for protecting member’s rights, confidentiality of the member and medical record.
* · *Responsible for reporting all incidents to the chain of command supervisor first.
* · *Responsible for ensuring members follow their daily schedule and when indicated with their daily living task.
* · *Assisting members in exercise/wellness activities.
Such as following station rotations calendar.
* · *Knowledge of CPR as event by annual training documentation is submitted* · *Knowledge of First Aid* · *Responsible for implementing the Safety/Disaster Plan when applicable* · *Responsible for successfully completing job specific and annual training as indicated in the training plan.

(18 credit hours of post high school education in a field related to DDD completed nor more than four years before the date the individual begins providing DDD services and two years of full time DDD work experience or* · *Four years of full time DDD work experience* · *High School diploma prefer at least one year of behavioral health field experience, but willing to train with a GED or Hight School diploma* · *Must possess and maintain a current AZ Driver’s license and are responsible to report any changes in their driver status if/when applicable* *PHYSICAL & MENTAL REQUIREMENTS:* *MUST HAVE VALID FINGERPRINT CLEARENCE CARD* · *Lifts 50 pounds* · *Drives a vehicle.

* *WORKING CONDITIONS & EQUIPMENT OPERATION:* · *Able to use computer, fax, and Telephone* · *Ability to create and edit daily prog notes* · *Abel to drive the facility vehicle* · *Able to use kitchen equipment* · *Able to stand on feet for long periods of time* · *Ability to type using word, excel and Therap tablets.
* · *Requires going in and out of facility several times a day.
* · *Physically, mentally, and actively participate with the members in indoor/outdoor activities such as gardening seasonal outdoor activities, taking walks, community activities.
* · *Must be able to communicate effectively with all parents, guardians, school staff for pick up and drop off changes, current events etc.
* · *Drivers will be responsible for Agency phones when completing routes and community outings.
