*Summary:*
We are looking for a Patient Navigator that will sit out of our Advanced Fertility Care (AFC) Chandler Clinic.
This will be a full-time, full-benefits, 40 hours a week position.
Must be available Monday- Friday, 7:30 am-4:30 pm.
We are looking for an energetic and compassionate Patient Navigator to join our team! As Patient Navigator you will provide remote support to patients in our clinics across the country through their fertility journey.
In this position, you are expected to possess exceptional communication skills, a passion to serve, and a proven track record of organization skills.
You are expected to collaborate with the clinical staff (by telephone, or via e-mail) in a timely and accurate fashion, the goal is to streamline communication, educate, schedule, and document all aspects of the patient treatment plans.
*Responsibilities:*
* Support the IVF team members with all aspects of patient treatment coordination, such as patient communication and education, scheduling, documentation, and record-keeping
* Assist and interact with patients in person, by telephone, and via email/text
* Collaborate with team members to deliver a high-quality patient experience
* Utilize nAble EMR system to ensure patient records are up-to-date and secure
* Provide support to the team with other administrative tasks as necessary
*Requirements:*
* Bachelor’s degree Preferred (Pre-Med Bach Degree is ideal)
* Ideal candidate will have Women’s Health or IVF Coordinating Experience
* EMR experience
* Excellent time and people management skills with the ability to work independently, multi-task, and prioritize work.
* Social perceptiveness and excellent service orientation
* Excellent written and verbal communication skills.
* Proficiency in MS Office.
* Must have reliable transportation and be willing to travel to other offices as needed.
* Bilingual - Spanish (preferred not required)
