*Job Summary for a Population Manager:* The Population Health Manager is an expert in value based primary care measures as well as leveraging technology to support a large population of patients in need of care.
The Population Health Manager is responsible and accountable for supporting clinical efforts to close care gaps for complex patient populations.
This role will perform supporting clinical disease management, assessment of disease states and utilization, care plan development and facilitation and referral to appropriate levels of care in addition to overseeing a team of Medical Assistants, Outbound Calling Representatives and other associates as needed to accomplish objectives.
*Qualifications for a Population Health Manager*
* High school diploma or equivalent
* 3 years of experience in a value-based primary care setting managing care gap closure for large quantities of patients
* Microsoft Excel and EMR experience proficiency required; (Epic EMR a plus, but not required)
* Possess strong verbal and written communication skills
* Excellent interpersonal and customer service skills
* Bilingual in English and Spanish preferred
* Have a passion for healthcare and an entrepreneurial spirit
Job Type: Full-time
Pay: $60,000.
00 - $80,000.
