*Education:* * Graduated from an approved school for Nurse Practitioners *License(s):* * AZ State License * Valid AZ DEA with Schedule II- IV DEA *Experience:* * Evaluation & management for chronic pain conditions * Familiarity with Surgical Intervention for Musculoskeletal Diagnosis/Conditions * Epidurals, MBB, RFA * Spinal Cord Stimulation * Vertebral Augmentation * Vertiflex * MILD * Medication Management with Controlled Substances & Medications * Examines patient and performs a comprehensive physical exam, and complies medical data including health history and results of physical examination * Administers or orders diagnostic test, lab tests and interprets test results for deviations from normal * Develops and implements patient management plans, records progress notes and assists in provision of continuity of care Job Type: Full-time Pay: $100,000.
