The Patient Care Coordinator Lead oversees the comfort and engagement of the patients in the center during their stay for stabilization.
The Leads is responsible to ensure that Patient Care Coordinators are trained to assist the patients and clinical teams in maintaining calm and therapeutic milieu.
They assist the nursing team in obtaining both vitals and point of care testing.
They intervene in the administration of Seclusion and Restraint as appropriate.
The PCC Lead is a member of the shift leadership team that manages the units and patients.
ESSENTIAL JOB FUNCTIONS Provides leadership to unit staff, in collaboration with the Charge Nurse to maintain the safety and comfort of the milieu.
Intervenes with escalated individuals using therapeutic techniques for de-escalation and performs restraints/seclusions when needed to ensure the individual's safety and the safety of others.
Leads staff through de-brief of such interventions to promote best practice Assigns other Patient Care Coordinators their responsibilities for the shift and provides any needed direction to meet the needs of the unit during their shift.
Lifts, moves and transports individuals, using proper body mechanics or lifting devices for accident prevention.
Assists individuals with activities of daily living to include eating, bathing, dressing, toileting and transferring.
Conducts and documents individual's vitals and reports all questionable results to the RN/Physician attending the individual.
Conducts security screening and inventories property of individuals upon admission to the facility Conducts and documents all BAL, urine drug screens and urine pregnancy tests.
Documents all services and significant individual interactions in the electronic health record.
Monitors and documents patients Q30.
Responsible for maintaining and ensuring the cleanliness of the facility and designated assigned unit Other duties as assigned Will attend supervision sessions as required.
Will complete all other required trainings as assigned.
Depending on setting, may also: Float among various clinical services, where qualified and competent SUPERVISORY RESPONSIBILITIES Reporting Structure: This position reports directly to the PCC Manager Supervisory Functions: BHT/PCC report to the PCC Lead.
POSITION QUALIFICATIONS Minimum Qualifications High School Diploma or equivalent of 3 years of behavioral health experience (1 year on observation unit) Preferred Qualifications Associate's Degree in Psychology or related field and 1 year of behavioral health experience on an observation unit Bachelor's Degree in Psychology or related field Must have a Level-1 Fingerprint Clearance Card or have the ability to obtain one through DPS.
Certified training (Safe Clinch, CPR First Aid, therapeutic options, etc.
) Physical Demands Occasionally required to stand Occasionally required to walk Continually required to sit Occasionally required to use hands Continually required to talk or hear\ Occasionally required to lift/push light weights (less than 25 pounds).
Specific vision abilities required for this job include: close vision, distance vision, peripheral, depth, and ability to adjust or focus.
